The team company or organization
and team captain are responsible for ensuring that
all 21 crew members and alternates have read the
Release,
Waiver and Indemnity Form and agree to the
terms by signing the form or the Waiver
and Release of Liability Signature Page.
Crew members who are over 14
years of age but under 18 years of age on race
day must have a Waiver
and Release of Liability Signature Page signed
by a parent or guardian.
Any team members
who are not able to sign the Waiver
and Release of Liability Signature Page for any reason (new
addition, out of town participants, etc.) must
submit a signed Release,
Waiver and Indemnity Form to the CEO Foundation office.
Captains must submit the Waiver
and Release of Liability Signature Page to
the CEO Foundation office prior to their team practice.
Waivers for team members who signed the signature
page
should be kept
on file at the company/organization of the team.
Original copies of faxed Release,
Waiver and Indemnity Form can also be kept
on file at the company/organization.
Captains will be required
to sign a final roster on race day verifying
the
identities and receipt of waivers for all
team members.
All
Team Waiver Signature forms or Waivers of Liability
Release forms must be received BEFORE your first
on-water practice.

Team
members who have not signed their waivers may not
participate in practices and will not be allowed
to participate in racing. All team members who
have signed a waiver will receive a wristband to
wear during the festival. Anyone without a wristband
will not be allowed in a boat. Wristbands will
be distributed to Team Captains.
The names may change
as time goes on, and if so, the paddler’s
waiver must be sent to CEO Foundation, 121
S. Meramec Ave., 9th Floor, St. Louis, MO
63105
before
the practice

CEO Foundation must

Adjustments can be made to rosters
at the Captain's Meeting on race day. Captains
will be required to verify identities and
receipt of acknowledgements by signing the
final roster before they receive the team
wristbands.
